The accused reported he was increasingly anxious because of graduation in five days. He reported no prior mental health history and was on no medications.'

His NCO Non Commissioned Officer in Charge (NCOIC),' Master Sergeant Adkins, reports that on being confronted by his supervisor for missing morning formation, Manning 'began screaming uncontrollably and clenching his fists. His eyes bulged in his face contorted. He screamed three times and stopped, caught his breath, and collected himself.' Neither Manning nor the mental health provider found any evidence of significant mental health problem. Manningwas invited to return only if his OIC desired after JRTC.

The mental health provider described Manning as 'presenting almost rigidly physically and emotionally through out the discussion, and no other probable cause for his being sent is developed. His demeanor is reflected as his perhaps being perceived as odd by others, although there does not appear to be anything diagnosable about it.' Manning reports that both parents were alcoholics and since separating from his mother, he has had to rely on himself for survival, and for that reason very carefully manages his interactions with others. Manning is assessed with 'personality disorder specified mild with cluster (c) characteristics-- excessive but not compulsive.'

The September 29, 2009 assessment found that Manning 'had adjustment difficulties typical of persons in their twenties, which he was quite consciously exploring and engaging. He was doing fine. The mental health provider anticipated one or two more meetings prior to the accused deployment.'

Bradley Manning is command referred to FOB Hammer Combat Stress Clinic for a command-directed evaluation due to anger outbursts over the past year and a half-- where he shoved a chair and began yelling at his NCOIC. After he counseled him on the loss of a room key and yelled and flipped the table when counseled by his supervisors. Manning reports in that 2005 he was prescribed LexAprilo by a family physician for problems he was experiencing with his step-mother, and he was referred to a physician by his aunt in 2007 due to anxiety attacks he was having and wanted him put on LexAprilo. With respect to his current condition, Manning reports he was working the late shift with three other service members and that it was lonely. He reports persistent worry and anxiety about oversleeping and being late for duty and also a hypersensitivity to criticism of his mistakes. Manning is assessed with an 'anxiety disorder not otherwise specified, with a cluster (b)/(c) personality traits and insomnia.' Manning has a normal mental status examination. The Unidentified Mental Health Provider in Iraq No. 1, a psychologist, who performed the command referred behavioral evaluation determined Manning to be under a considerable amount of stress and that Manning did not have any social support system and was hypersensitive to criticism. The psychologist recommended that Manning be moved from night to day shift and that he be given low intensity duty for the immediate future. He also determined that Manning was a damnger to himself and others and recommended removal of his weapon or the bolt from his weapon.

Bradley Manning sends Captain Casey Fulton (married name Martin) an email comparing the published Collateral Murder video to the one found by Specialist Jirhleah Showman in folders that belonged to the fire section on the shared T-Drive and discussed by one other soldier and three or four officers. Around this time frame, Bradley Manning also burns a second CD-RW containing the aerial weapons team video. In order to made it appear authentic, he places a classification sticker and writes Reuters FOIA REQ on its face. He places the CD-RW in one of his personal CD cases containing a set of 'Starting Out in Arabic' CD's. He planns on mailing out the CD-RW to Reuters after he re-deploys , so Reuters could have a copy that was unquestionably authentic.

Bradley Manning has an alleged altercation with Specialist Jirhleah Showman. Prior to this incident, Manning is 'not accessed to be a risk of harm to himself or others' by mental health providers. Captain Casey Fulton is present and involved in the incident, but did not see when Bradley Manning is said to have struck Specialist Jihrleah Showman. Fulton informs Master Sergeant Adkins. Fulton said she believed the incident should have resulted in a DEROG, and that it was then Master Sergeant Adkins duty to report the incident. Manning is removed from the T-SCIF as a result of this incident and sent to work in the supply room of the 2nd Brigade Special Troops Battalion.

Bradley Manning is brought to the clinic by his NCOIC because he struck another soldier Specialist Jihrleah Showman 'in the jaw.' Manning discloses to the mental health provider that he desired to be opposite sex and was coming to grips about revealing that. He also reports that he openly with a wig as Breanna Manning for three days while on leave-- although not assessing Bradley Manning, 'as a threat or harm to himself or others,' the mental health provider placed Bradley Manning on unit watch and recommended the command bring the accused to the mental health clinic daily.

Bradley Manning's Unidentified Mental Health Provider in Iraq No. 3performed a behavioral health evaluation on Bradley Manning and opined that his emotion and behavioral symptoms continued to cause impairment.' Manning's 'progress or rehabilitation was considered poor and a separation under Army Regulation [AR] 635-200, Chapter 5-17 be initiated.' He considered making a recommendation as to Bradley Manning's access to classified information in his evaluation but did not do so because he had been informed that Manning was no longer allowed in the T-SCIF. Instead, he deleted the block referencing access to classified information on the MEDCOM Form 4038 in order to have more space to write notes on the form.
